Since we were founded as Hospice of Napa Valley in 1979, our South Jefferson Health Center has become a leading center for community-based care delivery and resources in the Napa County, for those facing transitions of aging, Alzheimer’s disease and other serious illness or the end of life. Formerly known as Collabria Care we are now part of the Providence health system, and our services include:
