Reach for Home works to end homelessness in Northern Sonoma County by connecting people who are unsheltered or at risk with housing programs, case management, and supports that promote self-sufficiency and permanent housing. Their services focus on rapid rehousing, permanent supportive housing, interim housing, and community partnerships to help clients gain stability.
